RSRS is not open at night, on the weekends or during civic holidays. An RSRS Medical Records Specialist will receive and respond to your request during regular business hours.
Emergency medical record search and retrieval services
are only available
from Monday to Friday:
8:30 a.m. to 5:00 p.m., E.S.T.
Please note:
- Your record may not be stored in the main RSRS storage facility. It may still be with the original doctor, if it has not yet been shipped to RSRS. If your doctor's practice closed more than a year ago, it is very likely that the record is stored at our distant long-term storage facility; it will take longer to retrieve your medical record(s) if moved to long-term storage.
- The fee for the copy and transfer of medical records is not covered by provincial medicare. The fee is set in accordance with guidelines and is determined based on the number of volumes. The fee is not prohibitive and RSRS works with patients to ensure that the information is accessible.
- An Authorization for Release of Information Form must be completed and signed by each patient requesting their record. Members of one household may use one form but still must sign where indicated.
- Each patient over the age of 16 must sign for their own record.
- RSRS acts as the physician's agent in facilitating the authorized release of patient information. Original patient records must be retained by the physician (or by RSRS on behalf of the physician).
- Please allow 4 - 6 weeks for delivery of your record from the date of confirmation.
