Maintaining the location of the practice along with the staff you have come to know and appreciate will be a wonderful way to transition for the patients.  We will continue with the normal office hours and protocol, as well we will continue to provide you with the caring and friendly environment you are already used to.

 

As requested by Dr. T. K. Tan, I would kindly request you to download and complete the required Ministry of Health Patient Enrolment Forms at your earliest convenience. You can fill out the forms print and mail them back.
