“It is with mixed emotions that I am writing to inform you that I will be closing my family practice. Dr. Tennent will be seeing patients until September 28, 2018. I will be available to refill prescriptions by phone/fax until October 26, 2018. I am leaving to pursue new career opportunities and, while I am excited to begin the next phase of my professional life, I will sincerely miss being your physician.
Finding a New Doctor
I have not been able to find anyone to take over my practice, therefore, it is important for you to begin seeking another healthcare professional to take over your care. Please visit the NLMA Newfoundland & Labrador Medical Association - they provide a list of doctors accepting new patients. Their website is http://www. http://nlma.nl.ca or by calling (800) 563-2003. Also, consider using your personal network to find a physician. A family member or friend may be able to ask if his/her doctor is taking new patients.
Your Medical Record
Your original medical record must be safely stored for a period of time before it can be shredded. I have arranged with RSRS, a physician-managed facility to safely store all records from my practice and assist you in obtaining a certified copy. There is a fee for the transfer of medical records. Your medical record is very important for the future of your healthcare. I strongly urge you to obtain a copy as soon as possible, regardless of whether you find a doctor immediately. It is important that you can access this information whenever it’s needed. To obtain a copy of your record(s), complete the form below.
Making the decision to leave my practice has not been easy, and leaving practice with so many wonderful patients will be the most difficult part. You have all taught me more than you will ever know. I have truly appreciated the opportunity to help manage your healthcare needs over the past 18 years. It has been a pleasure and a privilege.
