Effective October 1st, Dr. Danny Chan  invites you to join his practice located at 303 - 2175 Sheppard Avenue East  M2J 1W8,  a short drive from Dr. Sinukoff's office location.  Dr. Sinukoff has had several meetings with Dr. Chan and has every confidence in his knowledge and capabilities.  You also have the choice to switch to a doctor of your choosing if he/she is accepting new patients.

 

Dr. Danny Chan graduated from medical school at University of Toronto in 2016 after receiving his Bachelor of Health Sciences degree with honours at McMaster University.

 

He continued to complete postgraduate training in family medicine at University of Toronto at North York General Hospital. He is the past co-chief resident at North York General Hospital and recipient of the Stan Bain M.D. Leadership in Family Medicine award. He is involved with various committee work including the Ontario College of Family Physicians and Quality of Care at North York General Hospital. His interests include comprehensive family medicine, hospitalist medicine, and integration of technology in health care.

 

Dr. Chan has active appointment at North York General Hospital in the Department of Family & Community Medicine with cross-appointment in the Department of Medicine. He is fluent in English and Cantonese.

 

In order for Dr. Chan or your new doctor to access your medical records and to obtain a copy of your medical records for yourself, you must order a copy of your medical records, see #1 below.   Also, you must complete, sign and return the Ontario Ministry of Health and Long-Term Care Patient Enrolment and Consent to Release Patient Health Information Form (Patient Enrolment form), see #2 below.

 

1) Medical Record Request Form

The Medical Record Request form must be completed to ensure your record is available to Dr. Chan. (Please note that the fee for preparation and transfer of your record is not covered by OHIP). You can begin the process by using this simple online form (or complete the paper form you received in the mail and send it in along with the Patient Enrollment form).

RSRS will access your record(s) and will send a copy(ies) to Dr. Chan and/or to you. If you are not enrolling with Dr. Chan you should still use the form to request a copy for yourself.

 

2) Enrolment with Dr. Chan

To secure a place in Dr. Chan’s practice, you must book an appointment for a meet and greet after October 1st and provide completed Patient Enrolment form(s) by March, 2019.

The Ontario Ministry of Health requires that the form be printed and signed. You may send in your completed Patient Enrolment Form at the time you return the signed Authorization Form or bring the completed Patient Enrolment Form to your first appointment. Complete one form for each adult family member (16 years or older) who wishes to enroll. Children and dependent adults can be added to a single form in section 2 of the form.  Please don't forget to sign the form before you choose one of the following methods to submit it:
