A Message from Dr. Melissa Kelley

Date of Practice Closure: Aug. 31st, 2018

 

“Dear Patients,

 

I am writing to inform you that my practice will be permanently closing on August 31st, 2018 as I am moving out of province.

 

If you had an appointment scheduled with me after that date it is cancelled.

 

A letter will be sent to your family doctor informing them that my practice is closed and that I am referring your care back to them. In many cases I have also included suggestions for any ongoing care or management if needed.

 

If you have any blood work, diagnostic imaging or other investigations done after the last week of August then you should follow up with your family doctor to review these results and they will arrange any further follow up if needed.

 

I am required to store all of my patient records for a period of time before they can be destroyed. You may obtain a certified copy of your medical record by contacting RSRS, a physician managed storage company, at 1 888 563 3732 Ext. 1, or by completing and submitting the form below.
