Instructions for patients of Dr. Mary Wong

• Dr. Mary Wong will no longer see patients after Dec. 13,2019
• Dr. Mary Wong has identified Dr. Lina Al-Imari, Dr. Aditi Lakhani, Dr. Ekta Lakhani, and Dr. Jacob Shin who will take on those patients of Dr. Mary Wong who wish to transfer to him.
• All of Dr. Mary Wong’s patient records will be going into safe and confidential storage on behalf of Dr. Mary Wong, who must retain the records for a number of years before they can be securely shredded.  RSRS is the provider of storage and transfer services.

​You will need to request a copy of your DIGITIZED RECORD from RSRS in order to transfer to Dr. Lina Al-Imari, Dr. Aditi Lakhani, Dr. Ekta Lakhani, and Dr. Jacob Shin, as his practice is paperless.

• Should you choose not to transfer to Dr. Lina Al-Imari, Dr. Aditi Lakhani, Dr. Ekta Lakhani, and Dr. Jacob Shin, Dr. Mary Wong strongly recommends that you obtain a copy of your record regardless, as this information is important for you and for your new doctor.  It’s also very important in the event that you are without any doctor for any period of time, as you will be the only one who can offer information about your current and previous healthcare.
• To begin the process of obtaining / transferring a copy of your medical record (to Dr. Lina Al-Imari, Dr. Aditi Lakhani, Dr. Ekta Lakhani, and Dr. Jacob Shin, to another doctor or to yourself), please fill in the form on the right.
