Date of Practice Change: September 30, 2019

 

  • Dr. Percy Rajchgot has now closed his practice at 2100 Finch and has transitioned to BoomerangHealth Clinic as of September 30, 2019.
  • Dr. Percy Rajchgot will be working fewer hours at BoomerangHealth. The records of those continuing with him will be / have been transferred to the new clinic automatically. BoomerangHealth is no longer accepting additional patients for Dr. Rajchgot.

  • After November 30, 2019, all of Dr. Rajchgot’s records pertaining to past patients, as well as those NOT registered with BoomerangHealth will be going into safe and confidential storage at RSRS, a physician managed records facility where they will be held for a number of years before they can be securely shredded. RSRS will also help you obtain a copy of your record for yourself and/or your new healthcare provider.
