Due to sudden health related issues I have had to make the very difficult decision to close my practice effective immediately.   My last day in office was Friday July 13th 2018.   The office will remain open until August 15th, 2018

You do not need to find a new physician in order to get your record.  I must store all patient records for a period of time before they can be shredded. I will be storing the records with RSRS, a physician-managed facility to safely store all my records from my practice and to assist you in obtaining a certified copy.  Your record is very important for your future care. Making the decision to leave my practice after 42+ years has not been easy,and leaving practice with so many wonderful patients will be most difficult. I have valued the trusted relationships we have developed, and I truly appreciated the opportunity to help manage your healthcare needs.
