Dear Patients:

 

After practicing family medicine for 45 years, it is with mixed emotions that I inform you that I will be retiring from my medical practice on October 1, 2018. I have been delaying my retirement until now because I have enjoyed my practice very much and wanted to ensure I have a transition plan for my patients before retiring.

 

I feel very fortunate in finding Dr. Leon Bhatia, MD, MRCGP, CCFP, to take over my practice. It is very gratifying to have found such a caring and competent doctor who shares my values and philosophy, to provide for your continuing care.  He is a 2007 graduate, and I think you will benefit from the transition to a younger, more energetic doctor.

 

This office location will remain the same at 855 Broadview Ave, Suite #405.  The telephone number 416 967 9220 remains unchanged and my secretary, Elizabeth who has been with me for 40 years will still be here. Your medical records will automatically remain with Dr. Leon Bhatia.

 

If you already have an appointment from October 1, 2018 onwards, you will be seen by Dr. Leon Bhatia. In order for Dr. Leon Bhatia to ensure the best possible continuity of care, you (plus any additional family members) must fill out the required Ministry of Health Enrolment forms, a blank copy of which can be downloaded by clicking here.

 

You can fill out the forms and mail them back using the included prepaid and preaddressed envelope. Children can be registered on the same form as their parents. Each adult in the household is required to fill out a separate form to register but you may photocopy the original form if additional copies are required. If you need another blank form, please call our office and our secretary Elizabeth will email you another form. Alternatively, you may fill out the forms in person at our office (with or without a doctor's appointment) and Elizabeth will assist you.
