Notice of Practice Closure

 

It is with great sadness we advise you of the sudden passing of Dr. Kuntz. The practice is now closed permanently as there is no other physician to take over. Dr. Kuntz will be greatly missed by his family, friends, patients, staff and by everyone who knew him.

 

It is strongly recommended that you obtain a copy of your personal medical history from RSRS. This information is important for your continued care and may not be available to you or your next healthcare provider otherwise.
