  • Dr. Sinukoff will no longer see patients after Sept. 28, 2018
  • Dr. Sinukoff has identified Dr. Danny Chan who will take on those patients of Dr. Sinukoff who wish to transfer to him.
  • All of Dr. Sinukoff’s patient records will be going into safe and confidential storage on behalf of Dr. Sinukoff, who must retain the records for a number of years before they can be securely shredded.  RSRS is the provider of storage and transfer services.

You will need to request a copy of your DIGITIZED RECORD from RSRS in order to transfer to Dr. Chan, as his practice is paperless.

  • Should you choose not to transfer to Dr. Chan, Dr. Sinukoff strongly recommends that you obtain a copy of your record regardless, as this information is important for you and for your new doctor.  It’s also very important in the event that you are without any doctor for any period of time, as you will be the only one who can offer information about your current and previous healthcare.
  • To begin the process of obtaining / transferring a copy of your medical record (to Dr. Chan, to another doctor or to yourself), please fill in the form on the right.
