When disability or death happens to a physician, the responsibility for the retention, notification, copying and distribution of medical charts usually falls on the doctor's spouse or adult child, who automatically becomes the Health Care Information Custodian, unless physician colleagues have agreed, in writing, to assume full responsibility for compliantly retaining the disabled or deceased doctor's records for the full term.
